What is the Automated Discharge Planner’s Program?

The Automated Discharge Planners Program was designed to create a win-win situation for all involved in the discharge of a patient or client to a Residential Care Facility for the Elderly (RCFE), also known as Assisted Livings or Board and Care Facilities.

The program was designed to cut down on the time a discharge planner spends finding bed availability for a patient. In turn, the patient and their family will receive immediate and appropriate options for their loved one. The RCFEs listed provide families with choices for care.

When a patient is in need of placement, the discharge planner or other health care professional will enter basic data on the patient regarding, budget, geographic location and care needs. The Automated Discharge Planner’s Program will immediately produce a list of rooms or beds available meeting the specified criteria of the patient.

The initial phase of the program is being beta-tested within the San Gabriel Valley area. Evaluation of the program will be conducted over the next 90 days, making your feedback very important in order to accomplish the goal of this program. An additional feature for discharges to skilled nursing facilities will be added in the second phase of the program, scheduled to be active by late 2008.

About Senior Solutions is a senior resource, evaluation, placement and advocacy organization dedicated to serving the needs of aging seniors and their family members.

The organization was founded by a professional woman passionate about providing families with answers to health care dilemmas. The Automated Discharge Planner’s Program offers a win-win solution to those involved with a patient’s discharge.

Who is putting this Program together?

This is a license protected program designed by About Senior Solutions to meet the urgent placement needs of seniors, their families and the cohesive relationships with the senior’s healthcare provider.

What do I need to do if my RCFE wants to participate?

For an RCFE to be involved in the program, they will be required to do 3 small things:

    1. Complete and Update a RCFE Facility Profile form- a form that contains information specific to your facility that is updated on an “as needed” basis to notify users of your bed availability.
    2. Complete a 2008 Referral Fee Contract—a form indicating the contractual agreement for placement between the RCFE and About Senior Solutions.
    3. Attend a brief, free training at Verdugo Hills Hospital, Huntington Hospital or Citrus Valley Health Care Partners. The training will provide the RCFE with further explanation on the program, onsite technical training, the ability to enter a profile via the internet, and explanation on how to maintain your updates and Profile information.

What if I miss a training date for the Automated Discharge Planner’s Program?

If you miss a training date available, or a training is full, please check the website, www.aboutseniorsolutions for additional training dates or contact About Senior Solutions by email at info@aboutseniorsolutions.com or phone at 626-359-0108 to request to have your facility’s name added to the waiting list.

Who can use this program for discharges?

At this time, the program is designed for professionals to access instant bed availability in RCFEs. Every participating organization will receive a user password to access the system 24 hours a day, 7 days a week.

The program is scheduled to offer instant bed availability to skilled nursing facilities by late 2008.

Is there a fee to participate in the program?

At this time, there is no fee for healthcare professionals to utilize the Automated Discharge Planner’s Program. At this time there is no monthly fee for the RCFEs to participate in the ADP program, however, a 2008 RCFE Contract will be required.
